The Comparison of Corticosteroid Injections and Prolotherapy for the Treatment of Greater Trochanteric Pain Syndrome

Abstract
Purpose: This study compares the effect of local corticosteroid injections and prolotherapy for the treatment of greater
trochanteric pain syndrome.

Materials and Methods: From May 2008 to December 2011, we divided 137 patients of greater trochanteric pain syndrome to local corticosteroid injection group and prolotherapy group. Effects of treatment were compared with visual analogue scale (VAS), Harris hip score and Roles and Maudsley score at pre-treatment, 1, 6, 12 months and post-treatment during the follow-up examinations.

Results: Before the treatment, Average VAS were 6.2 and 5.9 in local corticosteroid injection group, and prolotherapy group respectively. The average VAS improved to 2.1 and 4.3 after 1 month of treatment. After 12 month of treatment, VAS in prolotherapy group was improved to 2.0 whereas VAS in local corticosteroid injection group worsened to 4.9, compared to the result of 1 month after treatment. Average Harris hip scores were 64.3 and 61.0 respectively before the ttreatment, 92.3 and 83.4 after 1 month of treatment and 80.3 and 95.7 after 12 months of treatment. The proportion of the patient who showed excellent or good Roles and Maudsley score of local corticosteroid injection group was significantly higher, 67.7%, after 1 month of treatment, but in 12 months after treatment, prolotherapy group was higher, 83.3%, than local corticosteroid injection group.

Conclusion: Because prolotherapy, as a non-surgical treatment for greater trochanteric pain syndrome, showed longterm
therapeutic effect and it is considered to be more effective treatment than the local corticosteroid injection.
